If you are planning on attending the convention as either a regular attendee or as a student attendee, here are two versions of the registration form:

If you are planning on participating in the Golf Tournament, here are two versions of the tournament registration form:

If you are having trouble with any of these files, here are some suggestions:
1. Make sure that you are running Adobe Acrobat version 5.0 or later;
2. Right-mouse-button click on the link and save the .pdf (or .doc) file to somewhere that you can find it (e.g. 'Desktop', c:\temp...), and then launch the file from your local machine;
3. These are forms, they are designed to be filled out by typing your information onto the form, using the 'tab' key, 'arrow' keys or mouse to move between the fields, and then printing the completed form.
